Pacientes acometidos por trauma da face: caracterização, aplicabilidade e resultados do tratamento fonoaudiológico específico

Patients with facial trauma: characterization, applicability and results of an specific speech treatment

Resumo

Objetivo:  caracterizar principais sintomas e sinais clínicos de pacientes acometidos por traumas da face encaminhados para reabilitação fonoaudiológica após tratamento emergencial, e verificar os resultados do tratamento fonoaudiológico específico proposto para esses casos.

Métodos:  18 pacientes consecutivos encaminhados para avaliação fonoaudiológica, por apresentarem trauma de face, constituíram os sujeitos da pesquisa. O levantamento dos sintomas e sinais clínicos foi realizado a partir dos Protocolos de Anamnese e de Avaliação Fonoaudiológica para Trauma de Face (Bianchini, 2004). O tratamento fonoaudiológico variou de 8 a 12 sessões seguindo o Protocolo de Tratamento de Fraturas (Bianchini, 2004) e modificado na presença de fraturas múltiplas ou acometimento de tecidos moles. Após tratamento específico os pacientes foram reavaliados. Os dados pré e pós-tratamento fonoaudiológico foram registrados por meio de vídeo e foto, tabulados e analisados por meio de estatística descritiva.

Resultados:  média de idade de 21,8 anos, sendo 78% do gênero masculino. Quanto aos fatores etiológicos, prevaleceram acidentes automobilísticos e motociclísticos. Quanto à caracterização das fraturas verificou-se maior incidência de casos envolvendo côndilo. Principais sintomas em ordem decrescente: dor na musculatura facial e/ou cervical, cansaço e redução de força ao mastigar, alteração da oclusão, limitação da abertura da boca, limitação e desvios dos movimentos mandibulares e ruído articular. Principais sinais clínicos funcionais: alterações em funções estomatognáticas decorrentes do trauma e desvios e/ou alterações no percurso dos movimentos mandibulares em 100% dos casos; limitação da amplitude desses movimentos em 88,8%; edema, dor à palpação e alterações musculares decorrentes da fratura em 61,1%; ruídos articulares em 44,4%; alterações cicatriciais em 11,1%. Na reavaliação, após intervenção fonoaudiológica, constatou-se: alterações musculares decorrentes da fratura em 38,8%; desvios e/ou alterações no percurso dos movimentos mandibulares, ruídos articulares, dor à palpação e alterações em funções estomatognáticas em 16,6%; edema e limitação da amplitude dos movimentos mandibulares em 11,1%; alterações cicatriciais em 5,5%.

Conclusão:  os principais sintomas e sinais miofuncionais orofaciais em casos de traumas de face referem-se à dor na musculatura facial e/ou cervical, comprometimento da mastigação devido à limitação e assimetria dos movimentos mandibulares, além de ruído articular levando a desconforto. O resultado do tratamento fonoaudiológico específico proposto para esses casos mostrou-se eficiente, uma vez que todos os pacientes apresentaram melhoras importantes em relação aos aspectos alterados, minimizando as seqüelas inerentes aos traumas e viabilizando o funcionamento do sistema estomatognático.

Abstract

Purpose:  to characterize the symptoms and clinical signs of patients with facial trauma who had indication for speech rehabilitation after emergency treatment and to verify the results of the specific speech treatment proposed for these cases.

Methods:  18 patients sent for a speech evaluation due to facial trauma. The investigation of the symptoms and clinical signs was conducted through Anamneses and Speech Evaluation Protocol for Facial Trauma (Bianchini, 2004). Speech treatment varied from 8 to 12 sessions, following the Treatment Protocol for Facial Trauma (Bianchini, 2004) and was modified when multiple fractures were present or when soft tissues were involved. After the specific treatment, patients were re-evaluated. Pre and post-treatment data were registered using video recordings and photographs, tabulated, and analyzed using descriptive statistics.

Results:  the group’s mean age was 21.8 years, 78% were male. Automobile and motorcycle accidents prevailed with etiologic factors. As for the characterization of the fractures, a higher incidence of cases involving the condyle was verified. The main symptoms in a decreasing order: facial muscles and/or cervical pain, fatigue, and strength reduction during mastication, dental occlusion alterations, limitation, and deviation of the mandible movements and presence of noise at the mandible joint. The main functional clinical signs: alterations of the stomathognatic functions due to the trauma and deviations and/or alterations of the mandible excursions in 100% of the cases; amplitude limitations of these movements in 88.8% of the cases; edema, pain during palpation and muscle alterations due to fractures in 61.1% of the cases; noise at the mandible joint in 44.4% of the cases; scarring alterations in 11.1% of the cases. In the re-evaluation, after speech intervention, the following was observed: muscle alterations due to fracture in 38.8% of the cases; deviations and/or alterations of the mandible excursions, noise at the mandible joint, pain during palpation and alterations of the stomathognatic functions in 16.6% of the cases; edema and amplitude limitation of the mandible movements in 11.1% of the cases; scarring alterations in 5.5% of the cases.

Conclusion:  the main myofunctional symptoms and signs in facial trauma cases are related to facial muscles and/or cervical pain, alterations in the mastication due to limitations and asymmetry of the mandible movements, besides noise at the mandible joint which leads to discomfort. The result of the specific speech treatment proposed for these cases demonstrated to be efficient, since all of the patients presented better results regarding the altered aspects, diminishing the side effects of traumas and promoting a better functioning of the stomathognatic system.
