Territorial distribution and quality indicators of compulsory Neonatal Hearing Screening in Brazil after Law 12,303/2010

Abstract

Purpose: to survey the national literature after the universal neonatal hearing screening became compulsory, addressing aspects related to its territorial distribution, the screening procedures employed, and verify whether the internationally proposed quality indicators have been achieved.

Methods: an integrative review of articles indexed in the BIREME, PubMed, and Scopus databases. Inclusion criteria: Articles in Portuguese and English published after Law 12,303/2010 was passed, whose theme was the neonatal hearing screening in Brazil. Exclusion criteria: Information from books and/or chapters, integrative review articles or reflexive articles, studies conducted in other countries, and studies approaching a specific subpopulation. The search strategy combined the following descriptors in Portuguese and English, respectively: “Triagem Neonatal”, “Perda auditiva”, “Recémnascido”, “Brasil”, “Neonatal Screenings”, “Hearing Loss”, “Newborn”, “Brazil”.

Results: of the 224 articles found, 26 were duplicates, 38 were books or book chapters, three were integrative reviews, 20 were studies conducted in other countries, and 120 were either reflexive articles or studies approaching a specific subpopulation. The final sample comprised 17 articles that addressed the proposed theme.

Conclusion: the South and Southeast Regions of Brazil concentrated most of the programs, and the main technique employed was the evoked otoacoustic emissions. Two quality indicators were achieved: performance rates within the first month of life, and diagnosis referral rates. However, the coverage rates remained below the expectation, and there were still high retest referral rates, high nonattendance rates in the subsequent stages, low satisfactory retest result rates, and a large number of “fail” in the two screening stages.

Resumo

Objetivo: realizar um levantamento da literatura nacional após a obrigatoriedade da triagem auditiva neonatal universal abordando aspectos relacionados à sua distribuição territorial, aos procedimentos adotados para a triagem, assim como verificar se os indicadores de qualidade sugeridos internacionalmente foram alcançados.

Métodos: revisão integrativa com pesquisa de artigos indexados nas bases de dados Bireme, PubMed e Scopus. Critérios de inclusão: artigos em português e em inglês publicados após a sanção da Lei 12.303/2010 e que apresentassem como tema a Triagem Auditiva Neonatal no Brasil. Critérios de exclusão: informações de livros e/ou capítulos, artigos de revisões integrativas ou artigos de reflexão, estudos feitos em outros países e estudos referentes a uma subpopulação específica. Para a estratégia de busca, foi utilizada a combinação dos seguintes descritores em português e em inglês, respectivamente: “Triagem Neonatal”, “Perda auditiva”, “Recém-nascido”, “Brasil”, “Neonatal Screenings”, “Hearing Loss”, “Newborn”, “Brazil”.

Resultados: dos 224 artigos encontrados, 26 eram duplicatas, 38 eram livros ou capítulos de livros, três eram revisões integrativas, 20 eram estudos realizados em outros países e 120 eram artigos de reflexão ou estudos referentes a uma subpopulação específica. A amostra final, foi composta de 17 artigos que abordaram o tema proposto.

Conclusão: as regiões Sul e Sudeste concentram o maior número de programas, e a principal técnica foi a das emissões otoacústicas evocadas. Em se tratando dos indicadores de qualidade, os dois alcançados foram os índices de realização em até um mês de vida e o de encaminhamento para o diagnóstico. Entretanto, as taxas de cobertura continuam aquém do esperado, e ainda há índices elevados no encaminhamento para reteste, grande evasão nas etapas subsequentes, baixo índice de resultado satisfatório no reteste e grande número de “falha” nas duas etapas da triagem.
